Obituary for Carolyn Sample Carolyn Sample, age 62, passed away on Saturday, December 15, 2018, at Vanderbilt University Medical Center in Nashville, TN. Carolyn was born in Chillicothe, Texas, on October 17, 1956, to the late Elmo Wayne Johnson and Ethel Evelyn Teague Johnson. She previously worked in housekeeping and loved traveling, walks in the park and spending time with her family.

Carolyn was preceded in death by her parents; brother, Johnny Johnson and sister, Beverly Roberts. She is survived by her fiance’, Thomas Ferguson; son, Bobby Eugene Blaylock; brothers, Jimmy Johnson (Dixie), Darrell Johnson, Wally Hobson (Ginger); sisters, Linda Panter (Jacky), Jackie Olvis (Michael), Sheila Kellogg (Troy); grandson, Mason Sheppard; and many other family members. A celebration of Carolyn’s life will be Thursday, December 20, 2018, at 2pm in the Pegram Community Center, 453 Thompson Road, Pegram, Tn.
