OBITUARY Denise K Boyd March 2, 1945 – December 13, 2018 Denise Karen Boyd (Czosnyka), age 73, passed away peacefully on Thursday, December 13, 2018 in Phoenix, AZ after a short battle with cancer. She was born March 2, 1945 at St. Anne’s Hospital, Chicago, IL to Harold (a Sergeant in the U.S. Army) and Irene “Gerry (Brzoza) Ptacek. She graduated from the College of Dupage Glen Ellyn, IL in 1974 as a Registered Nurse.

She dedicated her life not only to her children, but to healing the pain and suffering of others for 44 years. She was an avid football fan, who could be heard loudly cheeing for her grandson’s teams and all the football teams coached over the years by her son, Rob. Denise will be missed by all in the football stands.

Her life will forever be cherished in the lives of her children: Robert (son) and Dwan Czoznyka of Peoria, AZ, Scott (son) and Trish Czosnyka of Peoria, AZ, Kim (daughter) and David Streetman of Ellaville, GA. Her grandchildren: Brevin, Antonio, Zachary, Madison, Carson, Conner, Kasey, Maggie, and Mary Kate; her great-grandchild Quinn. Family, friends, and others whose lives Denise has touched are invited to Sunland Memorial Park 15826 N Del Webb Blvd.

, Sun City, AZ from 4-7pm on Wednesday, December 26, 2018 for a visitation. Funeral Services will be held at Sunland Memorial Park Lakeside Chapel on Thursday, December 27th at 9am with a graveside service to follow.
