Beloved husband of Melissa J., nee Hartmann; cherished father of Jayson and Jessica; loving son of Leonard and Linda, nee Lepore; dearest brother of Michelle (Mike) Gerulis; dear nephew of James (Renee) Lepore; fond cousin of Amber (Bryan) Parkhurst and A.J. Lepore and second cousin of Charlotte and Maddie. Jay will be missed by all of his family and friends, but especially, his canine companion, Alvin.

Jay was an avid Chicago sports fan, but a diehard White Sox fan. Visitation Wednesday, June 13, 3 – 9 p.m. at Anderson Funeral Home, 21131 W. Renwick Rd. , Crest Hill.
