Polca and the late Henry M. Polca; grandmother of Maggie Ray, Brody Ray and Anakyn Ray Smith; sister of Sherry Bea Back (Collett) of TX, Sidney A. Collett of FL, Delton (Paula) Collett of VA, Duane (Jennifer) Polca of KS, Michael (Ronda) Polca of FL, and Denise (Thomas) Patrick of PA.

A Memorial service will be held on Sunday, 1-3 PM in Thomas J. Gmiter Funeral Home, Inc. 2323 E. Carson St., Pittsburgh, PA. 15203 and an additional service will be held on Monday, 3:30 till 4:30 PM in Cranberry Place, 5 St. Francis Way, Cranberry Twp. , PA.
