OBITUARY Jerry Lee Hernandez May 3, 1972 – December 11, 2018 Jerry Lee Hernandez, age 46, of Chicago, Illinois passed away on Tuesday December 11, 2018. Jerry was born May 3, 1972. Jerry is survived by; son Anthony Ruiz; son Jesse Hernandez; and daughter Alexis Hernandez; mother Elvira Villanueva.

Proud grandfather of Jayden and Emma Grace. Mr. Hernandez also leaves his 2 sisters: Cindy and Blanca; many nieces, nephews, cousins, aunts and uncles to cherish his memory. Jerry was preceded in death by; father John Hernandez.

Visitation Thursday, December 20, 2018 from 2:00 to 8:00 PM at Mount Auburn Funeral Home, 4101 S. Oak Park Avenue, Stickney, IL 60402 (708) 749-2033.
