Johnny Jeremiah leaves to cherish his memory his parents, Johnny Guidry, Jr. of Opelousas, LA and Kesha Semien, of Port Barre, La, siblings, Keon Keitaro Guidry, Keelan Denzel Guidry, Lakayasha Landrio, Johnny Guidry III, Jaleel Guidry, maternal Grandmother, Helen Semien of Port Barre, LA, paternal grandfather, Johnnie Guidry, Sr. of Opelousas, LA;Uncle, Joseph Semien, Surprise, AZ; Aunts, Taraca Miller (Gabriel) of Carencro, LA, Marilyn Semien of Opelousas, LA, Vonda S Guidry, Opelousas, LA, and a host of cousins, other relatives and special friends.

Johnny Jeremiah was preceded in death by his paternal grandmother Mary L. Guidry and maternal grandfather Joseph Charles Semien, great-grandparents, Carlton and Joyce Sam, cousin Destiny Pickney, great Aunt Clara Sam, great-uncle Joseph Malveaux, paternal great-grandparents Morris Chenier and Cora Chenier Thompson.
