She departed this life January 2, 2019 at her home in Salesville, Arkansas at the age of 69 years, 5 months and 22 days. She spent her life as a loving daughter, sister, wife, mother, grandmother great grandmother, homemaker and friend.

Se was a member fo the Pleasant Valley Community Church in Calico Rock. She is survived by one son: Richard Cook of Salesville; one sister: Kathy Mayfield of Salesville; two grandsons: Blake Cook and wife Sarah and Logan Cook and wife Danielle all of Springfield, Missouri; one great grandson: Wyatt Cook of Springfield; one brother-in-law: Jerry Branscum of Calico Rock and a host of other relatives and friends. She was preceded in death by her parents; two brothers: Charlie Mayfield and Johnie Mayfield and two sisters: Cynthia Branscum and Becky Lamb.

A private memorial service is planned at a later date.
