Carmen Hernandez (Perez), surrounded by her family, entered into eternal rest on Saturday, June 9, 2018, at her home. Of New York City, NY, she was born in Camajuani, Cuba. Carmen is survived by her loving family, her son, Teodolo Yzquierdo and her daughters Ana Sanchez, Yolanda Yzquierdo and Maria Hernandez.

She is also survived by her cherished grandchildren, Tatiana Ponce, Grisel Palacios, Julio Sanchez, Miguel Yzquierdo and her sister Dora Perez. Visitation will be on Monday, June 11th, from 4 – 9 PM. Funeral Mass will be offered on Tuesday, 10:15 am, at St. Joseph of the Palisades R.C. Church, West New York, NJ.

Entombment to follow at St. Michael Mausoleum, East Elmhurst, NY.
