Monte Hale Formy-Duval, 71, passed away Friday, May 17, 2019 at Duke University Hospital in Durham, NC He was born in Columbus County, NC on March 30, 1948, the son of the late James Cecil and Emma Gore Formy-Duval. He was preceded in death by his former wife, Mary Ann Brown Formy-Duval and also a brother, J.C. Formy-Duval. Mr. Formy-Duval had retired as a Valve Tech with Nuclear Power Plants.

He was a member of Pleasant Plains Baptist Church. Surviving is his wife, Donnie Hooks Formy-Duval of the home; two sons, Coval Formy-Duval of Whiteville, NC and Pierre Formy-Duval and wife, Julia of Tabor City, NC; granddaughter, Isabella Formy-Duval; two step-sons, John Hooks and wife, Karen of Wilmington, NC and Phillip Hooks and wife, Kristen of Ocean Isle Beach, NC; two brothers, Jack Formy-Duval and wife, Faye of Chadbourn, NC and Gene Formy-Duval and wife, Joyce of Boiling Springs Lake, NC. The family will receive friends from 1:00 – 1:45 pm Monday, May 20, 2019 at Pleasant Plains Baptist Church with a funeral service at 2:00 pm.

Officating will Rev. Dave Heller. Interment will follow in the Western Prong Baptist Church Cemetery.
