Hazel was born in Norristown, Pennsylvania on Friday October 2, 1942, daughter of the late Russell and the late Emma Lou Mims. Hazel has lived in Palm Coast, Fl for the last 9 years, formerly of Conshohocken, PA.

Bell Service will be held on Wednesday, October 17, 2018 at 11:00 AM at St. John A.M.E. Church 8th Avenue & Harry Street, Conshohocken, PA 19428. Viewing will be held on Wednesday morning from 9:00 to 10:45 AM at the Church. Services will be officiated by The Rev.
