Phyllis Jean Cook, 87, passed away March 2, 2019, at The Waters of Lebanon. She was born on January 21, 1932, in Indianapolis, to the late James Joseph and Eva (Caldwell) Fall. Phyllis was married to Arthur Cook; he preceded her in death.

She was employed with her cousin’s insurance agency as a secretary and was a very skilled typist. Phyllis enjoyed working crosswords and puzzles. She loved her family; especially spending time with her grandchildren.

Pitcher of Lebanon Scott Riley of California Jeremy Foncannon of Indiana Great Grandchildren: Ashlee Stapleton Taylor Stapleton Dakota Foncannon Aaron Pitcher Brant Pitcher Caitlin Bilskie Anthoney Riley Paige Foncannon Kayla Hunnicutt Great-Great Grandchildren: Hunter Graham Sophia Riley Lukas Bilskie Daisey Bilskie In addition to her parents and husband, Phyllis was preceded in death by a daughter, Rita Sue Foncannon, and two brothers, Myron Fall and Joseph Fall. Myers Mortuary and Boone County Crematory have been entrusted with her cremation arrangements. No visitation or service is planned.
