Hazel Cook Swann, age 94, of Douglasville, GA, passed away on Tuesday, January 15, 2019. She was born on March 5, 1924, in Atlanta, GA, to the late Bob & Jewell Cook, and was preceded in death by husband, Moody Swann. Mrs. Swann was a member of Church of Christ in Cartersville, GA.

Survivors include sons, Robert Swann of Dothan, AL, and Timothy Swann of Douglasville, GA; daughters, Gay & Tommy Prewett of Acworth, GA, and Patricia & George Sims of Dallas, GA; seven grandchildren, seventeen great-grandchildren, and many nieces and nephews. Funeral services will be held on Friday, January 18, 2019, at 2 PM, from Clark Funeral Home, with Minister Stan Wenck officiating. Interment will follow in Rosehaven Cemetery.
