THOMAS’ Obituary Thomas J. Chack, 65 years old of Salem, WI passed away on Friday, May 17, 2019 at the Hospice Alliance House, Pleasant Prairie, WI. He was born on May 6, 1954 in Wausau, WI the son of the late Stanley and JoAnne (Wood) Chack. He was the husband to the late Carol Chack.

Thomas loved being outdoors – especially when it came to hunting and fishing. He also loved being a father, grandfather, and brother. Thomas always lived life to the fullest, no matter what situation was thrown his way.

He was a great friend and someone to talk to. Survivors include his son, Timothy (Diana Branson) Chack; brother, Bob Chack; family member, Ashley Hubner, and grandchildren, Mikey, Christopher, Matthew, and Kole. He is preceded in death by his parents; sister, Sharon Durant; grandson, Michael, and stepson, Josh.
