Helen I. Ptak, 90, formerly of Fair Oaks, died Tuesday, June 19, 2018, in Good Samaritan Hospice, Beaver. Born November 24, 1927, in Uniontown, a daughter of the late Martin and Tekla Rejzer, she was a member of the Good Samaritan Church. In addition to her parents, she was preceded in death by her husband of 56 years, Leonard T. Ptak, on June 9, 2006; a son-in-law, Ray Fowler, and seven sisters.

Surviving are three sons and daughters-in-law, Kenneth and Lori Ptak, Colorado; David and Rebecca Ptak, Penn Hills, and Timothy Ptak, Pennsylvania; a daughter, Kathleen Fowler, Baden; 10 grandchildren, and 11 great-grandchildren. Friends will be received Thursday from 4 to 8 p.m. in the BOHN-MATICH FUNERAL & CREMATION SERVICES, INC. , 1099 Maplewood Ave.

, Ambridge, where a service will be held Friday at 9 a.m., followed by a Mass of Christian burial at 9:30 a.m. in Good Samaritan Church.
