Preceded in death by his mother, Charlotte and grandmother, Mary Murphy. Jack is survived by the love of his life, Teresa; loving sons, Jack and Michael; brother, Mike; sisters, Vickie (Jim) Risner and Becky (Dave) Wooten; Aunt, Patty; numerous extended family and friends along with his devoted best friend, Joe Streitenberger, who was always by his side.
