She was born in Dayton, Ohio, on June 27, 1941, the daughter of Orwen Charles Dennis and Viola Hayes. Phyllis was the wife of Emilio Juarbe of Waterford.

Besides her husband she is survived by a son, Edward Juarbe (Amanda Brouwer-Juarbe) of Waterford, four daughters, Elizabeth Juarbe of New London, Emily Juarbe-Rivera (Pedro Rivera) of Goshen, NY, Gloria Juarbe (Victor M. Moniz) of Philadelphia, PA, Victoria Rivera (Romualdo Rivera) o0f Brooklyn, NY; her brother Dana Dennis of Dayton, OH, 8 grandchildren, Holly Parkes, Katherine and Virginia Juarbe, Peter, Jared, Lauren, Ashley and Kaylee Rivera; and one great grandson Juda Parkes. She was predeceased by her brother, Richard Dennis and her sister Jennifer Varrisco.
