OBITUARY Hazel E Slayman September 3, 1919 – October 22, 2018 Hazel E. Slayman, 99 of Louisville, passed away Monday, October 22, 2018. She was preceded in death by her husband, Charles J. Slayman. Left to cherish her memory, her children, Charles R. Slayman & Dara Hilliard; 6 grandchildren, 9 great-grandchildren, and 3 great-great grandchildren.

A memorial visitation will be Thursday, October 25, 2018 from 2-4pm at Arch L. Heady & Son Funeral Home, 8519 Preston Hwy. , with a service to celebrate Hazel’s life at 4pm in the Arch L. Heady & Son Chapel.
