Jack Raymond Johnson was born in Iowa City, IA on April 29th, 1951 to Ilene and Rex Johnson. He passed away peacefully on Friday September 7th, 2018 at the age of 67. Jack served selflessly in the Army and moved on to work with adults with disabilities.

He is survived by his daughter Mary and sisters Rose and Lorraine. He was preceded in death by his brother Steve, mother Ilene, and father Rex.
