Obituary for Carol Ann Homiak Smith Carol Ann Homiak Smith, 76, of Mentor, passed away peacefully, Saturday, Nov. 24, 2018, at Lake Health West Medical Center in Willoughby surrounded by family. Born Jan.

19, 1942, in Shamokin, Pa, she had lived in Cleveland for 70 years, before moving to Mentor six years ago. Carol was a devoted mother and grandmother. Survivors include her children, Shirley (Ronnie) Deskins, Debbie Hambleton, Jessica (John) Valyik, and Michael Smith; and grandchildren, Dan, Jennifer, Amanda, Railee, Erica, Eric, Samantha, Tabatha, Ben, Janet, and Sophie; 18 great-grandchildren; and sister, Janice (John) Belon.

It was Carol’s wish was to have a celebration of life party which is to be held from 3 to 7 p.m. Saturday, Dec. 1, 2018, at 38850 Sunset Trail, Willoughby, OH 44094. Arrangements are entrusted to McMahon-Coyne-Vitantonio Funeral Homes in Willoughby and Mentor.
