Obituary for John Ray Fields Johnny Fields, age 55, passed away Thursday, July 19, 2018 surrounded by his family. He was the custodial engineer at Columbus Ophthalmology Associates for 18 years. Avid Notre Dame football fan.

Preceded in death by his father, Robert Fields; brothers, Jody and Bobby Fields. Survived by mother, Beatrice Fields; sister, Debbie Fields; his children, Jaime Williams, Toni Williams, Gabby Slatzer (Josh DeLucco), Josh Williams (Nina) and Robby Slatzer (Keri); 7 grandchildren, Landon, Bryson, Erin, Keegan, Emrie, Luke and Makenna; nieces, Jodi, Rebekah (Wayne) Wright, Robin (Josh) Rohr, Beanca (Sam) Cooper, Ashlee (Anthony) Melick and Amanda Fields; nephews, Bobby and Shaun Fields; best friend for over 30 years, Kelly Williams; numerous great nieces, nephews and friends. Visitation will be Wednesday, July 25th from 4 until time of the service at 6:30 pm at the O. R. WOODYARD CO.

CHAPEL 1346 S. High Street. A memorial gathering will be held at the Four S Club 281 E. Whittier after the service. To send flowers or a memorial gift to the family of John Ray Fields please visit our Sympathy Store.
