She was born on August 30, 1962, the daughter of Jimmy Lee Smith and Sondra Kay (Ball) Smith. Phyllis was a waitress in several area restaurants and had delivered newspapers for the Enquirer and later the Shopper News.

She met her Donald while working at the Te-khi Truck Stop where they were married on October 13, 1999. Phyllis is survived by her Husband of 18 years, Donald Everett Nelson; Sons, Charles Gale, Benjamin Bax and David (Danielle) Nelson; Daughters, Angela Gale, Jamie (Ryan) Mills and BillieJo (Ryan) Hastings; 7 Grandchildren & 1 Great Grandson; Sisters, Cathy Tyler, Sandra (Joe) Cooper, Rusina Evon and Linda (Jose) Carmona; Brother, Phillip Smith. She was preceded in death by her Parents.

Phyllis enjoyed crocheting, needle point, knitting, crafts, scrap booking, video games frogs, her puppies, Bella Mae, Baby Girl, Bandi Sue and Buddy Boo Boo, her cat, Tom and her bird, Lucky Ann. She love spending time with her family, especially her grandchildren.
