Carmel West Hospital. Preceded in death by his father, Byron Booth. Survived by loving wife of 30 years, Tammie; sons, Jorden and Travis; siblings, Diane (Steve) Evans, Darla (Anthony) Clark; Danny, Dawn, and Dennis Booth; brothers-in-law, Clarence (Sue), Jeff (Deb), and Sean (Regina) Mayhugh; many nieces and nephews.
