Obituary of Douglas Cook Douglas D. Cook, 60, of Jamestown, passed away Tuesday (October 30, 2018) in Heritage Park Rehab & Skilled Nursing Facility. He was born March 9, 1958 in Jamestown, the son of the late DeForest W. and Nina Allette Cook. Douglas was a graduate of Jamestown High School, and had been employed with the Re Center.

He enjoyed being a volunteer with the SPCA. He is survived by his wife Mamie Terry Cook whom he married in June of 2000 in Jamestown; his Aunt Lois Weiss of Frewsburg. Besides his parents he is preceded in death by his grandparents Ned and Beatrice Cook, two aunts: Laura Straight and Sharon Almgren.

The funeral will be at 1 p.m. Thursday in the Falconer Funeral Home. Rev. Gerald Haglund will officiate.

Burial will be in Sunset Hill Cemetery. Friends will be received by the family from 12:00 p.m. to 1:00 p.m. Thursday in the funeral home. Memorials may be donated to the Humane Society-Chautauqua County 2825 Strunk Rd.
